Forty Under 40 Application

Please fill out the following survey to be considered for IB's Forty Under 40 (40 professionals under the age of 40). Our editorial team evaluates submissions once a year and looks forward to your submission!

Applications should be completed and submitted by the individual applying. If you have a good candidate in mind for the Forty Under 40, please forward them this online application form. IB must have a completed application form to consider a candidate. Third party nominations are not accepted.

PLEASE NOTE BEFORE FILLING OUT APPLICATION: If selected for the 2023 Forty Under 40 class, any information you include in your application (or within the reference letters) may be used in a brief (200 words or less) profile that will be published online at IBMadison.com and inside a feature article in the March 2023 issue of In Business magazine. IB will not get approval on these editorial profiles before they are published, so please do not include any information in your application (or in your reference letters) that you are not comfortable having appear in print. If you need a compliance department to review any information included about you in your printed profile, you MUST get approval from them ahead of time before you submit your online application.

2023 application deadline: Sunday, November 13, 2022. Applications received after that date will be kept on file for consideration the following year.
Applicants must be under the age of 40 on 3/1/23, and must work in the counties of Dane, Columbia, Dodge, Green, Iowa, Jefferson, Rock, and Sauk to be eligible.
